Abstract
The importance of multi-disciplinary team working was discussed in Chapter 3. This chapter explores the importance of dietary and medicines optimisation in the care of people with a long-term condition (LTC). Part One: nutritional management. The relationship between both obesity and malnutrition and LTCs is explored focusing on aetiology, associated health risks and management of both obesity and malnutrition. Practical guidelines are provided relating to assessment and management of a patient’s nutritional status, including why weight management advice is important. Part Two: medicines optimisation. It is estimated that 50% of patients with LTCs may fail to take their medicines properly. Online links to NICE quality standards and schemes promoting medicines optimisation are provided, along with definitions of the term. This section will also briefly discuss the role of non-medical prescribing in the management of LTCs.
