Several questions about the study design should be raised.Death was the end point in the study; yet it is not clear if the two study populations were comparable in cardiac risk. It was stated that patients were excluded if they had "other diseases that might be expected to influence mortality." The population under study, middle-aged men with chronic obstructive pulmonary disease, is one in which arteriosclerotic heart disease, clinically apparent or silent, is common.
