Abstract

The emergence of physiotherapy as an independent applied science enables the profession to redefine its diagnostic and therapeutic strategies. This paper examines aspects of musculo-skeletal physiotherapy within this context. It questions some of the principles on which the art and science of physiotherapeutic intervention is based, and argues that self-responsibility of the patient must be an integral aspect of this evolving management model. Current concepts in the role of rehabilitation and neuromusculoskeletal diagnosis are discussed, and intervention strategies examined. The emphasis on passive modalities is questioned in view of our increasing understanding of the physiology of the tissues and pain perception. Active involvement of patients in all aspects of their rehabilitation is encouraged in the context of promoting physiotherapists as specialists in rehabilitation, not simply experts in palliative care. It is argued that it is the collective responsibility of the profession actively to promote self-management.
