Abstract

One hundred and twelve biopsy specimens from 98 patients with lesions of necrobiosis lipoidica diabeticorum were reviewed. Three basic types of histopathologic changes were noted: (I) palisading granulomatous, (2) tuberculoid, and (3) intermediate. The palisading granulomatous type was characteristic of the lesions from patients with diabetes mellitus while the tuberculoid type was seen almost always in nondiabetic patients. Histologic features of the intermediate type were midway between those of the other two types and were seen more than twice as frequently in nondiabetics as in diabetics. The proposed classification emphasizes merging of the histopathologic reactions into one another and points up the likely unity of the disease in diabetics and nondiabetics.
