Abstract

A 43-year-old man sustained open fracture of his right tibia and fibula by a traffic accident during long term hemodialysis. At four months after the accident osteosynthesis was done by bone grafting and external fixation of Hoffmann's instrumentation. After admission, 1α (OH) D3 has been administered daily with a dose of 0.5μg. At four months after the first operation he revealed non-union and the second operation of osteosynthesis by a plate and screws with iliac bone grafting was performed. Bone union has been recognized on X-ray from six months after the second operation. Histomorphometric analysis of iliac bone was performed before and after the medical treatment by 1α (OH) D3 for four months period. It showed hisological features of osteomalacia and osteitis fibrosa, and it revealed that relative osteoid volume was decreased and percent labelled surface by tetracycline was increased without change on the number of osteoclast after administration of 1α (OH) D3.
