Abstract
当院で頸椎症性脊髄症に対して棘突起縦割式椎弓成形術を施行し，2年以上フォローアップ可能であった42例の術後アライメント変化について検討した．頚椎前弯角，頚椎前弯指数，頚椎可動域は術後徐々に減少する傾向にあった．頚椎形態は術前前弯型28例，直線型8例，後弯型6例が，術後前弯型24例，直線型12例，後弯型6例となった．JOA scoreの平林法改善率が50％以上を成績良好群，50％以下を成績不良群とすると，術後頚椎弯曲指数でのみ有意差がみられ，術後頚椎前弯位を保てている症例の方が成績がよい傾向にあった．C2付着部筋の処置の仕方でC2未操作群，C2温存群，C2修復群の3群に分けて検討したが，アライメント，JOA scoreに関して有意差はなく，C2付着部筋の修復が術後後弯変形を予防できる可能性があると思われた．術後に頚椎前弯位を保てるような治療計画をたてるべきであり，術後アライメントに影響する因子の解析を更に進めていく必要がある．
