Abstract
症例は92歳の女性．胸痛と発熱を主訴に当院救急外来を受診した．CT検査で縦隔リンパ節炎を疑い，ドリペネムの点滴静注を開始し入院加療とした．抗菌薬投与に反応を示さず，採血上炎症の増悪及びCT検査で心囊液の増量を認めたため，心膜開窓ドレナージ術を行った．手術所見では心囊内が膿性心囊液で満たされており，心囊内に連続する縦隔に膿瘍腔を認めた．心囊液の培養からはHeamophilus aphrophilusが検出された．以上の経過から縦隔リンパ節炎穿破に伴う化膿性心外膜炎と診断した．化膿性心外膜炎は致死的な疾患であるにも関わらず，最善の治療についてのコンセンサスは未だに得られていない．抗菌薬不応である場合，心囊開窓術は治療選択肢として十分考慮できると考えられた．
