Abstract
光線力学的療法（photodynamic therapy; PDT）とは，腫瘍親和性光感受性物質とレーザー光線との併用にて悪性腫瘍内で光化学反応を惹起させ，腫瘍組織を選択的に死滅させる治療法である．皮膚科では，ポルフィリン前駆体の5-aminolevulinic acid（ALA）を外用しレーザーを照射するPDTが皮膚疾患の治療に有効である．外用されたALAは，腫瘍組織内に取り込まれてヘム合成経路を経て，光感受性を示すプロトポルフィリンIXに生合成される．PDTは表在性皮膚悪性腫瘍の日光角化症，ボーエン病，表在型基底細胞癌の治療に最も効果がある．われわれの成績では，1－2回のPDTにてcomplete responseは日光角化症で82/98病変，ボーエン病にて20/35病変，表在型基底細胞癌にて3/4病変であった．表皮内浸潤の乳房外（外陰部）Paget病や菌状息肉症の治療にも応用される．腫瘍内に蓄積した光感受性物質の蛍光発生を利用する光線力学的診断（photodynamic diagnosis）は境界不鮮明な腫瘍の輪郭の確認や，腫瘍再発の発見などの腫瘍の局所診断に有用である．外用ALA-PDTは，非侵襲的で安全で，美容的にも優れた治療法である．各皮膚腫瘍の適切な治療をめざし今後さらに検討が必要である．
