Abstract
5例のPancoast型肺癌を対象に超音波断層法を施行し, 超音波モニター下の経皮針穿刺診を施行した.超音波断層法は肩甲部において前額断, 矢状断などの画像が得られ, 本腫瘍と鎖骨下動静脈, 筋, 腕神経叢などとの関係を連続かつリアルタイムに観察しえ, 穿刺術のガイドとして腫瘍の局在を観察する上で有用であった.超音波ガイド下針生検, 吸引細胞診の陽性率は, それぞれ92.9%, 46.7%であり, 両者の併用で全例に悪性所見が得られた.合併症は認めなかった.一方, 喀疾細胞診, TBLB, 気管支鏡下キュレットによる陽性率は, それぞれ16.7%, 0%, 22.2%と低く, 超音波ガイド下経皮針穿刺診はPancoast型肺癌の病理検体を得る方法として, 安全かつ確実な方法であった.
