Abstract
近年，周術期抗凝固の普及などに伴い，硬膜外鎮痛が使いにくい状況が多くなってきている．一方で患者自己調節鎮痛（PCA）を用いたオピオイドの全身投与は硬膜外鎮痛の代替手段として使用頻度が増加しているものと思われるが，副作用の発生が懸念される．このような状況の中で末梢神経ブロックが急速に普及しており，さらに非オピオイド鎮痛薬や鎮痛補助薬を積極的に使用するなど，作用機序の異なる鎮痛手段を組み合わせた多様性鎮痛（multimodal analgesia）が注目されている．本稿では最近の術後痛管理の動向を解説し，さらに安全で効率的な術後痛管理としてチーム医療に基づいた術後痛管理システムを紹介する．
