Abstract
悪性腫瘍や放射線障害により尿管浸潤あるいは尿管狭窄をきたした症例に対して尿管を一部切除し，有茎腸管による尿管の再建を行った11例（計15回）を対象として合併症，長期予後について検討した．対象症例の原疾患は，直腸・S状結腸癌4例，子宮癌・肉腫4例，卵巣癌1例，尿膜管癌1例，消化管外GIST 1例であった．代用尿管として再建に使用した腸管は回腸9例，S状結腸6例であった．術後短期の合併症は4例に認められ尿路感染症3例，水腎症2例，癒着性イレウス2例，代謝性アシドーシス1例，腹腔内膿瘍1例であった．術後平均観察期間は16.3カ月で，その間反復する尿路感染症はなく術前と比べた術後の腎機能低下は認められなかった．代用尿管として腸管を用いる本術式は，骨盤内病変による尿管切除後の再建方法として有用であり，治療の選択肢のひとつとして考慮されるべき術式と考えられた．
