Abstract
嚢胞性病変を含む前中縦隔の良性腫瘍の切除方法は, 開胸術か肋間からのアプローチによる胸腔鏡下手術で行われることが一般的である.今回我々は, 剣状突起下アプローチにて胸骨吊り上げを行い, フレキシブルスコープを用いて胸腺嚢胞を摘出したので報告する.症例は80歳男性.乾性咳噺を主訴に当科を受診.胸部造影CT, 胸部MRIて前縦隔左側に44×42×38mm大の薄壁単胞性の嚢胞性病変を認めた.胸腺嚢胞を疑い, 剣状突起下アプローチによる胸骨吊り上げ胸腔鏡下手術により嚢腫を摘出した.病理組織学的に胸腺嚢胞の診断を得た.前中縦隔の嚢腫性病変の摘出に対して本アプロ-チは有用であると考える.
