Abstract

It is often difficult to treat the intra-articular fractures of the distal radius, especially unstable type. In our department, we treat and restore the unstable intra-articular fractures of distal radius with arthroscopic technique. Fourteen wrist joints of 13 patients were examined in the present study. These cases were classified as follows: Colles fractures (8 cases), Smith fractures (3 cases) and central depression types (3 cases).This surgical therapy was indicated in those with the unstable fractures of IVb and IVc types according to modified Gartland classification. Although there existed more than 5mm gap of joint surface before the surgery, the gap was corrected and reduced to less than 1mm after the surgery. We adopted Saitoh's point system to evaluate the treatment effects. All the cases were regarded as more than good level. We took the palmar approach in addition to the conventional dorsal approach to make arthroscopic examinations. The palmar approach was particularly usefull when we identified a dorsal fragment in the intra-articular fracture of distal radius.
