Abstract

A 55-year-old female with multicentric cancer of the hypopharynx visited our hospital complaining of right upper cervical lymph node swelling. Clinical and pathological investigation showed a right small piriform sinus tumor. She was treated by irradiation (60Gy).Local recurrence was detected nine months later, so total laryngopharyngoesophagectomy with total thyroidectomy and bilateral neck dissection and reconstruction with a free flap of jejunum was performed. Microscopic evidence of multicentric origin was demonstrated in serial sections of the excised tumor. Dysplastic epithelium was found surrounding the cancer at varying distances.We emphasize the importance of a wide margin in the resection of cancer of the hypopharynx.
