Abstract
患者は27歳男性で, 4年前に Marfan 症候群に伴う annuloaortic ectasia に対して Bentall 手術を受けた. 今回突然の腹痛, 腰痛をきたし当科を受診した. 腹部に圧痛を伴う拍動性腫瘤を認め, 腹部超音波検査にて腹部大動脈の拡張を認めた. 腹部大動脈瘤破裂を疑い, 緊急手術を施行した. 開腹すると最大径6cmの紡錘状の腎動脈下腹部大動脈瘤を認めた. 瘤を切開すると壁在血栓はなく, 瘤壁は薄く, 後壁に約2cmの破裂孔を認めた. Knitted Dacron 製人工血管を用いて腎動脈下腹部大動脈と両側総腸骨動脈間で置換し, 吻合部は wrapping 補強し, 下腸間膜動脈も再建した. 大動脈瘤壁の病理組織学的所見は中膜壊死, 変性が観察され, Marfan 症候群に一致するものであった. Marfan 症候群に伴う腹部大動脈瘤はまれなものであるが, 急速に増大し, 破裂の危険性が高く, 厳重な外来経過観察と迅速な診断のもとに早期手術を行うことが重要であると考えられた.
