Abstract
発見時に両側鼠径リンパ節および多発骨転移を伴っていた早期胃癌を経験したので報告する.症例は78歳の女性で全身倦怠・食欲不振を主訴とし, 右第8肋骨の突出と両側鼠径リンパ節の腫大を認め, 血中carcinoembrionic antigen 30,880ng/mlと非常に高値であった.胃内視鏡で幽門部にIIa+IIc様隆起, 体部に粘膜下腫瘍および潰瘍を認め, 生検は幽門部のみgroup VでCEA染色陽性であった.骨シンチで多発性の異常集積が認められた.幽門側胃切除と右鼠径リンパ節生検を行い, sm早期胃癌, 粘膜下腫瘍はleiomyoma, 潰瘍はU1-III, 鼠径リンパ節も胃癌からの転移と判明した.早期胃癌の4群リンパ節転移は珍しく, 中でも鼠径リンパ節転移の報告は2例のみであった.骨転移についての報告も非常にまれで, とくに胃癌発見時に存在した例は剖検例を含め8例目であった.早期に転移を来す要因の解明が待たれる.
