Abstract
We present the case of an 11-year-old boy who complained of right tinnitus and dizziness provoked by loud sounds and by playing a pipe after a minor right-sided head injury. Pure tone audiometry revealed conductive hearing loss, and the acoustic reflex test was negative in his right ear. His eye movements were then recorded by electronystagmography during loud sound stimulation. With pure tone sound stimulation at 800 Hz/110 dBHL and at 4000 Hz/110 dBHL to the right ear, a left-beating nystagmus was recognized. It was hypothesized that a dislocation of the stapedo-vestibular joint with a subluxation of the base of the stapes into the vestibule, together with hyperexcitability of the inner ear, had occurred during the minor head injury. Therefore, loud sounds stimulated the vestibule and caused the subsequent dizziness.
