Abstract
頚椎後縦靭帯骨化症の兄弟家系の検索の過程で頚椎後縦靭帯骨化を認めた二卵性双生児姉妹を発見しその臨牀経過において興味ある知見を得たので報告する．症例は69歳の二卵性双生児姉妹でありいずれも低身長であり依存症として統合失調症があった．姉は頚椎に連続型の頚椎後縦靭帯骨化がありその骨化占拠率は48％であったが脊髄症状は全く無く自立して生活していた．妹は非常に酷似した頚椎後縦靭帯骨化を認めたが骨化占拠率は45％であるにも拘わらず重度の脊髄症状を認め介護施設に入所していた．頚椎後縦靭帯骨化症における脊髄症状発現機序についてはまだ不明な点があり，静的圧迫因子のみでは説明がつかない．本症例でも遺伝的因子は近似し骨化の程度もほぼ同じなのに臨床経過は異なっていた．このような症例の集積は本症の脊髄症状発現機序解明につながると考える．
