Abstract
Until today, there are no universally accepted methods for detection of unstable atherosclerotic plaques, even though many recent studies were devoted to this issue. In this article we present modern possibilities of computed tomography in visualization of atherosclerotic coronary lesion, including the detection of unstable lesions, whot in turn, can help in diagnosing subclinical exacerbation of ischemic heart disease and in the stratification of risks of acute coronary events.

Быстрое увеличение АСБ после кровоизлияния может вызвать гемодинамически значимое сужение просвета или увеличение объема некротического ядра и разрыв АСБ. Для разорванной АСБ характерно наличие крупного некротического ядра, занимающего, как правило, более 30 % площади АСБ, тонкой фибринозной покрышки менее 65 нм, а также тромба, выступающего в просвет КА через разрыв в покрышке, визуализирующегося как продолжение ядра [9, 25]. Что при наличии ядра размером менее 3 мм и с площадью менее 1,0 мм, а также процентным соотношением ядра к остальным структурам менее 10 % и толщиной покрышки менее 150 нм АСБ является стабильной и вероятность ее разрыва низкая [5, 15, 29]. Для выявления атеросклеротических поражений с риском разрыва используются другие КТ-признаки: положительное ремоделирование КА, низкая плотность (менее 30 HU), наличие точечных кальцинатов размерами менее 3 мм, «кольцевидное усиление» рентгеновской плотности по периферии АСБ, не превышающее 130 HU, а также неровный контур [30]
