Multifocal Polypseudophakia With Rayner Sulcoflex Trifocal and In-the-Bag Alcon PanOptix Diffractive Trifocal Intraocular Lenses: A Case Report

Abstract

Purpose: To describe the use of a sulcus diffractive multifocal intraocular lens (IOL) used in series with an intracapsular diffractive multifocal IOL to improve uncorrected near visual acuity (UNVA). Methods: A 66-year-old pseudophakic woman with bilateral multifocal IOLs presented with symptoms of negative dysphotopsia and poor reading vision in the right eye. A diffractive multifocal sulcus IOL was inserted in the right eye. Results: After 1 month, the patient's right UNVA had improved from N10 to N6. Subjectively there was a 50% improvement in negative dysphotopsia and no increase in photic phenomena. Conclusions: A diffractive trifocal secondary sulcus IOL can be used in series with an intracapsular diffractive trifocal lens. The success of this approach suggests that it may be used to both improve negative dysphotopsia and address postoperative refractive error.
