Abstract

Objective To investigate the MRI features of adult pilocytic astrocytoma in fourth ventricle and to improve the diagnostic accuracy. Methods The clinical data and MRI manifestations of 6 cases of adult pilocytic astrocytoma in fourth ventricle confirmed by pathology were retrospectively analyzed. Results Two cases were solid mass and four cases were solid cystic mass. All cases were in company with obstructive hydrocephalus, two cases presented with hemorrhage, and none of them had peritumoral edema. The solid part of the tumors showed hypointensity on T1WI and DWI, hyperintensity on T2WI. The cystic part of solid cystic mass showed long T1 and T2 signal, hypointensity on DWI, hyperintensity on FLAIR. The solid part were remarkably enhanced in the following contrast administration while the cystic wall and cystic part showed no enhancement. Conclusions Adult PA which occurred in the fourth ventricle is rare, and most of the tumors are solid cystic. It has some characteristic imaging features, which might be helpful for marking the differential diagnosis before operation.
