In this study, we seek to identify factors, especially those that may have socioeconomic implications, associated with patient no-shows in our county’s academic urology clinics. METHODS: A retrospective chart review was conducted on patients who were scheduled at the main urology clinics of our academic center between April 2018 and 2019. The total number of appointments scheduled and patient attendance were computed at each clinical site. Patient attendance was further analyzed according to type of scheduled appointment, type of clinician seen, and socioeconomic status based on the patient’s insurance coverage and domicile represented as Health Zones. Chi-square test and simple and multiple regression analysis were performed in order to determine the relationship between patient no-show and the variables for appointment characteristics. RESULTS: A total of 1,472 (13.8%) of the 10,709 scheduled appointments were no-show patients. Statistical significance was established between patient no-show and the multiple appointment and socioeconomic variables. There was a higher no-show rate for non-private faculty clinic than private faculty clinic (15.1% vs 9.0% respectively; p<0.0001), and appointments booked with advanced healthcare providers (16.5%) compared to with urologists (12.6%) (OR=1.20, 95% CI 1.05-1.38; p<0.0278). On regression analysis, no-show clinic patients were strongly associated with those who had no health insurance (OR=30.00, 95% CI: 23.73-37.92; p<0.0001) and those who reside in our county’s Health Zone 1 (OR=1.96, 95% CI:1.63-2.37; p<0.0001), in which the population reports a household income of 30% below poverty level and is only one-third high-school educated. Overall, the cohort of patients who are at risk for no-show are scheduled at the non-private faculty clinic with either no insurance or public insurance and reside in health zone 1. CONCLUSIONS: Identifying the cohort of patients who are at risk for no-show allows for implementation of changes to this specific target population, which may help curtail the number of future patient non-attendance to urology appointments.
