Abstract

cancers in patients with a history of kidney cancer, predominantly in Caucasian populations. We sought to evaluate this association and identify clinical and pathological characteristics of these patients in a large urban, inner city medical center. METHODS: We reviewed our cancer registry data from 2000 to 2013 for patients diagnosed with kidney and prostate cancer. We identified patients who had a primary thyroid cancer diagnosed at any point using ICD-9 codes and reviewed their clinical records. We compared this potential association within the genitourinary cancer cohort. As prostate cancer is the most common genitourinary cancer, we compared the association of thyroid cancer with kidney cancer to that of thyroid cancer with prostate cancer. RESULTS: A total of 1374 kidney and 5874 prostate cancers were diagnosed during this 14-year interval, and a total of 18 patients had thyroid cancers (10 kidney; 8 prostate). We observed 5.3-fold increased risk of thyroid cancer in patients with kidney cancer over prostate cancer (p<0.0001, c21⁄421.864), which persisted when limiting the analysis to men (see table). Of the 10 patients with kidney cancer, histologic types included clear cell carcinoma in 4, papillary and clear cell histology in 2, papillary in 1, sarcomatous in 1, and unknown in 2. Thyroid cancer histologies included papillary (3/10), follicular (2/10) and anaplastic carcinoma (1/10) and unknown in (5/ 10). Notably, all patients with papillary thyroid cancers (3/10), had distant metastasis. The ethnicities of the patients with kidney and thyroid cancer were Caucasian (5/10), Black (3/10) and other (2/10). The longest interval between the diagnoses of the two cancers was 137 months. CONCLUSIONS: We observed a significant risk for thyroid cancer among patients with a history of kidney cancer. Interestingly, these thyroid malignancies appeared more clinically aggressive than typical reports of well differentiated thyroid cancer. Further investigation of this association is warranted.
