Abstract

‘Always supported, never alone’ is the motto of a new breed of worker who links up with patients who have dementia and their families via GP surgeries. Their job title is primary care dementia practitioners (PCDPs), but Julie Anderson, clinical community manager with Cornwall Partnership NHS Foundation Trust, describes them as ‘conduits’. It is an apt description. Communication in dementia care is a complex business. As well as understanding and articulating the needs of patient and carer, a host of other agencies that have the potential to improve lives must be marshalled and directed. Everything hinges on that effective conduit: someone who can channel information in all directions to benefit the person who matters most.
