Abstract

Renal transplantation has been proven to be the best modality of renal replacement therapy in patient with end stage renal disease. Due to the lack of legislation of deceased donor transplantation and sometimes unwillingness for donation or unavailability of suitable donor, patients travel abroad to get commercial transplantation. In some part of the world it is been carried out illegally in covert facilities. Commercial Kidney Transplants outcomes and complications carry a high morbidities & mortalities along with the heath care burden. Its outcomes are poorly documented in Oman. Aim of this study is to explore the trend, clinical characteristics and outcomes of Commercial kidney transplantation among Omani patients in the period between 2006 and 2016. A retrospective study passed on patients’ medical records. Eligible subjects are those patients, aged 13 years and above who went abroad for commercial living unrelated renal transplants and returned to our institution for post-transplant care. Sixty patients satisfied the entry criteria; 40 males and 20 females with age range 13-73 years, and mean ± SD of 42 ±15 years. Primary renal disease was unknown in 22 (36.7%) allograft recipients. The majority (73.3%) of the transplants were carried out in Pakistan. One-year post-transplant course was complicated by numerous surgical and/or infection complications. The rate of biopsy-proven rejection at 1 year was 30%. Patient survival for the study group at 1-year was 93.3% and allograft survival was 85%. Besides the ongoing ethical arguments, commercial kidney transplantation caries a complex post transplantation course. Overall One-year patient and allograft survivals are good.
