Abstract

Objective To study the modified large craniotomy for decompression of frontal,parietal,temporal and occipital lobes for the treatment of patients with severe brain injury with bilateral mydriasis.Methods Modified large craniotomy for decompression of frontal,parietal,temporal and occipital lobes were performed in 26 patients of severe brain injury with bilateral mydriasis before surgery.This method was compared and analyzed with the American standard large trauma craniotomy introduced by JIANG Ji -yao.Results 9 patients died after 1 week,2 died after 2 -3 weeks,and the last 15 were awake gradually.In the 15 cases,the longest was awake after 58 days and the shortest was after 15 days. The average duration was 26.6 days.10 cases were recovered well,5 were severe disability.Conclusion The modified large craniotomy for decompression of frontal,parietal,temporal and occipital lobes is more sufficient and effective than the standard large trauma craniotomy for the treatment of patients with severe brain injury with bilateral mydriasis.
