Abstract

Objectives We provide a description of a novel modification of a mucosal-sparing technique using electrocautery and balloon dilatation in the endoscopic management of web-like benign airway stenosis. Study Design We conducted a retrospective chart review. Setting We studied an outpatient bronchoscopy suite in a tertiary-care medical center. Participants Participants consisted of 3 patients with symptomatic, benign tracheal or bronchial web-like stenosis. Interventions Patients were treated with radial electrocautery ablation and balloon dilatation (REA-BD) through flexible bronchoscopy. Measurements and Results Three patients underwent 4 procedures. All treatments resulted in an improved airway cross-sectional area, pulmonary function (3 patients), and symptoms. Recurrence was seen in 2 patients, although only one was symptomatic and responded well to repeat treatment. Conclusions REA-BD is technically feasible and can offer significant advantages over traditional treatments of benign web-like stenosis in terms of cost, avoidance of general anesthesia, and training requirements.
