Abstract
Objective To analyze the middle-term effects of marrow core compression with thick passagc on early osteonecrosis of femoral head (ONFH).Methods A total of 46 adult patients with 56 hips suffered from the early stage ONFH underwent the surgery of marrow core decompression with thick passage,male of 32 cases,female of 14 cases,the mean age was 32.6 (20-53) years old.According to the etiology classification:the alcohol-induced ONFH was in 30 patients with 36 hips,sterorid-induced ONFH in 11 patients with 13 hips,steroid and alcohol-induced ONFH was in 5 patients with 7 hips.According to association research circulation osseous (ARCO) classifying,26,13,8,9 hips were in stage Ⅰ-A,Ⅰ-B,Ⅰ-C and Ⅱ-A,respectively.There were 43 hips in left side and 13 hips in the right side.Results The pain of the hips in all patients were relieved after the operation.The period of postoperative follow-up studied from 4 to 6 years.The results showed excellent in 48 hips and good in 8 hips.Of them,all hips were excellent in stage Ⅰ-A and Ⅰ-B (P < 0.05).There were excellent in 5 hips and good in 3 hips in stage Ⅰ-C.There were excellent in 6 hips and good in 3 hips in stage Ⅱ-A.The rate of excellent and good results was 100% (26/26),100% (13/13),62.5% (5/8),66.7% (6/9) in stage Ⅰ-A,Ⅰ-B,Ⅰ-C,Ⅱ-A.The best results was seen in the stage Ⅰ-A.In the thick passage decompression,the X-ray films showed obviously,there was a bone pillar shadow formed in the thick passage day by day,the avascular necrosis area were not collapsed.Conclusions The middle-term effects of marrow core compression with thick passage are satisfactory,especially in ARCO Ⅰ-A,Ⅰ-B stage patients.
