Abstract

<h3>To the Editor:—</h3> In the Question and Answer, "Transference of Responsibility" (<i>JAMA</i><b>188</b>:196 [April 13] 1964), an answer is given by Dr. Scher which should be seriously disputed. The problem concerned management of a 40-year-old woman who, despite considerable psychiatric care (of an unspecified nature), is still not able to function outside a protective setting. In the world, "she keeps very irregular hours, neglects her house and personal hygiene, and soon uses sedatives in dangerously large amounts." This scanty history provides little of the information so essential for diagnosis and management of such a severe emotional disruption of function. Dr. Scher advances a somewhat questionable and controversial answer, which, in essence, reflects an almost medieval concept, dwelling largely on the implication that this patient consciously is being manipulative and destructive to the people arourd her, and that "clear-cut responsibility demands" and "progressively enhanced realistic burdens" should be imposed upon
