Abstract
Introduction/Objective: The US healthcare faces major challenges in transitional care of patients. Residency programs around the nation are tackling this issue by implementing formal transitional care curricula. Most of these curricula focus on discharge summaries, medication reconciliation and post discharge follow up. Resident training regarding post-acute care (PAC) venues and reimbursement of their services is another area of transitional care that requires formal training. The purpose of this survey was to learn how Internal Medicine faculty rate first year house staff's knowledge about PAC services.
