Abstract
CME Educational Objectives 1. Delineate the changing epidemiology of disease states in children and describe model systems for chronic illness care and prevention. 2. Identify some of the controversies surrounding the medical home model and the critical components necessary for successful transformation to the medical home model. 3. Explain how state and federal policy must change in order for the medical home model to flourish and succeed in the near future.
