Meckel’s diverticulitis with neuroendocrine tumor

Abstract

A 61-year-old man with no significant medical history presented with abdominal pain, diarrhea, and fever. On presentation, he was hypertensive at 220/95 mmHg, tachypneic with respiratory rate of 30, tachycardia at 120 beats/min, and febrile at 40.1°C. On examination, his abdomen was soft with diffuse tenderness in all quadrants and minor distension. The white cell count was elevated at 14×109 mm/cell, with C-reactive protein of 180 mg/L and a venous lactate of 2.1 mmol/L. Computed tomography of abdomen and pelvis demonstrated inflammatory stranding surrounding a 2-inch-long blind-ending structure arising from the small bowel approximately 2 feet from the ileocecal valve with associated extraluminal gas (Fig 1).
