Abstract
This paper describes the application of a Safety Attitude Survey Questionnaire and a Process Safety Management System (PSMS) audit tool at six major hazard sites in four European countries. The tools provide quantitative measures of safety attitudes and PSMS performance respectively. The work compares these quantitative measures with accident performance data for the six sites. The work also considers the use of generic failure rate data used in quantified risk assessment for major hazards within the context of land-use planning around the major hazard sites, and the variation in accident performance data and safety attitudes of different sites.
