Abstract
大網裂孔ヘルニアは内ヘルニアの1つで，まれな疾患であり，特徴的な臨床所見に乏しく術前診断は困難とされてきた．2010年から2013年の間に当科で術前診断にMD-CTが有用であった6例の大網裂孔ヘルニアを経験した．年齢は58～83歳（平均65.3歳）で，男性5人・女性1人であった．MD-CTの画像上の特徴として，横行結腸の腹側に位置する拡張した小腸，closed loop，腸間膜や小腸の収束像が有用な所見であった．開腹歴のないイレウス例では大網裂孔ヘルニアを念頭に置く必要がある．MPRを用いたMD-CT検査が有用であり，大網裂孔ヘルニアを診断できる可能性がある．
