Abstract

Diltiazem is a calcium antagonist used in the treatment of angina, hypertension and some tachyarrhythmias. It has negative chronotropic and dromotropic effects on the sinoatrial and atrioventricular nodes, and is negatively inotropic. It also reduces systemic vascular resistance by arterial smooth muscle relaxation. There is limited experience in the management of a massive diltiazem overdose that can be fatal. 1–3 The present case, in which the patient survived, involves one of the largest diltiazem overdoses described.
