Abstract

<p>Background: Transitional care interventions for adolescents with congenital heart disease aim to prepare adolescent for transition from pediatric to adult healthcare settings.</p> <p>Purpose: To identify the components, mediums of delivery and dosage of transitional care interventions for adolescents living with congenital heart disease and map the identified intervention components to specific health-related quality of life domains.</p> <p>Methods: Arksey and O’Malley’s framework was used to identify transitional care interventions and map interventions to health-related quality of life domains.</p> <p>Results/Discussion: Eleven articles were selected for review. Intervention components included education, skill development and information about adult healthcare settings. Mediums of delivery included written and online resources, transition events, patient passports and transition coordinators. Dosage of interventions were not clearly reported. Mapping indicated that all reviewed transitional care interventions addressed at least one domain of HRQOL.</p> <p>Conclusion: Future investigations should address the relationship between transitional care interventions and health related quality of life.</p>
