Abstract

The authors retrospectively evaluated the success of and timing for intervention in retinopathy of prematurity. Forty eyes with stage 3 ROP were treated with cryotherapy or laser therapy soon after the detection of threshold stage and 42 eyes with stage 3 ROP were treated at a prethreshold stage. All patients had 4 months to 2 years of follow up. An overall unfavorable outcome (macular or disc drag or progression of ROP to stages 4 and 5) occurred in 14 (17.1%) of 82 eyes, 11 of which reached threshold during the critical period of 37 to 39 postconception weeks of age. Nine (22.5%) of 40 eyes treated in the threshold stage and 5 (11.9%) of 42 eyes treated in the prethreshold stage had an unfavorable outcome. Intervention at a prethreshold stage of ROP or at a younger conceptual age (ie, younger than 37 weeks) may decrease the incidence of an unfavorable outcome.—Michael D. Wagoner
