The amniotic fluid is clear. A limp, apneic infant is handed to the neonatal team. The infant is warmed, dried, and stimulated, and the airway is opened. Thirty seconds after birth, the heart rate is undetectable, and positive pressure ventilation is started at a rate of 60. The heart rate remains undetectable after 15 seconds of positive pressure ventilation; the team opens the mouth, repositions the head, and reapplies the mask.
