Abstract

The treatment for pain and dysfunction of extensor tendons and the distal radioulnar joint is a challenging component in the care of the rheumatoid upper extremity. The clinical manifestations of extensor tendon disease and surgical options for managing dorsal tenosynovitis, tendon nodules, attrition, and rupture are described. The techniques of tendon grafting and tendon transfer are delineated. Treatment alternatives for the painful distal radioulnar joint, including synovectomy, distal ulnar resection, hemiresection-interposition arthroplasty, matched ulnar resection, and the Sauve-Kapandji procedure, are detailed. The possibility of distal ulnar prosthetic replacement is discussed.
