Abstract

A significant number of patients with ulcerative colitis (UC) and the majority of patients with familial adenomatous polyposis (FAP) will eventually need colectomy. Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) is a preferred approach to proctocolectomy with permanent ileostomy, since the IPAA procedure preserves intestinal continuity and improves health-related quality of life. However, this bowel-anatomy-altering procedure is often associated with complications. The most frequently observed long-term complication of IPAA is acute and chronic idiopathic inflammation of the ileal reservoir, i.e., pouchitis. In this chapter, we provide up-to-date information on the etiology, diagnosis, and management of pouchitis.
