Abstract

Palmetto SeniorCare, located in Columbia, South Carolina, is one of the original replication sites for the Program for All-Inclusive Care. Through Medicare and Medicaid waivers, Palmetto SeniorCare (PSC) coordinates financing and delivery of all health, medical, and social services for a population of frail older persons. This article identifies ways in which this approach can enhance home-based care. PSC's methods of care planning and service delivery are described, and program utilization data and evaluation results are provided. PSC's relationship with home health agencies illustrates opportunities for collaboration and contracting in this form of man aged care. Key words: elderly persons, long-term care, managed care
