Abstract

A 76-year-old woman was admitted to the intensive care unit as a result of respiratory failure. A central catheter was inserted under ultrasound guidance via the left internal jugular vein. However, backflow appeared to be arterial, and the partial arterial O2 pressure was 97 mm Hg. A chest radiograph showed the catheter tip projecting laterally over the lung (Fig 1, arrow). Computed tomography showed the tip within the lung parenchyma (Fig 2, arrow). Venography through the catheter showed anomalous pulmonary vein draining into the left brachiocephalic vein through a vertical vein (Fig 3, arrow). The catheter was removed without consequences.
