Abstract

Primary cardiac malignancies are very rare compared with metastatic tumours to the heart. Owing to the rarity of these tumours and with their nonspecific symptoms, diagnosis is usually made during the advanced stage, where definitive management remains a challenge up to this point. We present a case of a 59-year-old man, who came in owing to chest pain and dyspnoea. Two-dimensional echocardiography revealed presence of a mass in the left atrium with the consideration of myxoma vs a thrombus. Excision of the tumour under cardiopulmonary bypass was done. Intraoperatively, a solid mass was found adherent to the left atrial wall and adjacent valve. Frozen section biopsy revealed a sarcoma. On final histopathology, the mass was found to be a malignant spindle and epithelioid sarcoma, making this the first reported case in the Philippines and in South East Asia. Seven months postoperatively, the patient remained asymptomatic; however, further follow-up for metastatic work-up was advised due to the aggressiveness and high recurrence of this tumour.
