Abstract

Malignant fibrous histiocytoma (MFH) is most common among adult soft-tissue sarcomas, although this disease occurs quite rarely in the lung. A case of MFH in the lung was reported with review of the Japanese literature. A 63-year-old male was pointed out the abnormal shadow on a chest X-ray film, and diagnosed as sarcoma after further examinations. Left 5th rib's thoracotomy was carried out, and it was found that the tumor occupying in S1+2 invaded the 2nd, 3rd and 4th ribs. Left upper lobectomy with resection of the chest wall was performed. The tumor macroscopically originated from the lung and was microscopically MFH. In spite of postoperative aggresive chemotherapy, the brain metastasis occurred 3 months postoperatively. The metastatic lesion was extirpated, and then CYVADIC therapy and whole brain irradiation were started. The patient has been under strict follow up up to now 16 months after lobectomy. The prognosis of pulmonary MFH is so poor that surgical treatment is the first choice.
