Abstract

Introduction: Reconstruction of soft tissue defects after temporal bone resection can vary from simple overclosure of the external auditory canal to complex flap closure. Large defects typically require reconstruction with locoregional pedicled myocutaneous or distant free myocutanaeous flaps. While microvascular free tissue transfer is an excellent option from a functional and cosmetic standpoint, patients often have significant comorbidities that increase perioperative risk during extended procedures. The pedicled lower island trapezius flap provides a reliable, cosmetically acceptable coverage in this patient population.
