Abstract
60歳以上の健常者用老人ホーム在住者 (n=313) の血清TSH, T3, T4およびTBG値と年齢との相関関係を検討した. その結果, T3およびTBG値と年齢との間にはそれぞれ有意な負の相関関係 (いずれもp<0.01) を認めたが, TSHおよびT4値と年齢との間には, 有意な相関関係を認めなかった. そこで, TSHおよびT4値に関しては全症例を一群とし, T3およびTBG値に関しては60, 70, 80および90歳の各年代を一群として, それぞれの平均値±2標準偏差を求め, それぞれの年代の基準値とした.この基準値を用いて, 1980年1月から6月迄の間に, 東京都養育院附属病院を受診した非甲状腺疾患症例 (n=127) の甲状腺機能を判定したところ, 69例 (54%) に低T3血症を見い出した. 低T3血症69例中62例は血清T4値が正常 (低T3群) であり, 7例はT4も低値 (低T3低T4群) であった. 両群の背景疾患に有意な差異を認めなかったが, 低T3低T4群においては, 低T3群あるいはT3正常群に比べ, Hb, 血清総蛋白および総コレステロール値が低値, かつ死亡率も高値であり, より重症な病態を反映していると考えられた.
