Abstract
A 24-year-old woman was referred to us with a trigeminal neuralgia refractory to medical therapy. Brain MRI disclosed a homogenously contrast-enhanced lesion located in Meckel's cave with a minor extension in the posterior fossa and impingement on the foramen ovale. The patient was operated on via a subtemporal interdural approach, and the mass was removed subtotally. Preoperative imaging and intraoperative macroscopic findings suggested a trigeminal schwannoma. The histological analysis showed no tumor, but inflammatory tissue compatible with sarcoidosis was seen. The neuralgia persisted and the patient underwent diagnostic workup for localized neurosarcoidosis. The patient underwent specific therapy with resolution of the clinical symptoms at 2 months from intervention. We will discuss this rare condition, which is difficult to diagnose.
