Abstract

686 Introduction: LURD kidney transplantation is still a controversial issue because of its clinical and ethical aspects. Aim of this study is to evaluate the factors affecting graft survival in our experience on LURD transplantation. Materials: from December 1983 to November 1998, 184 LURD kidney transplants at our institution were performed. The population was composed as follow: 155 "emotionally related" pairs- wife to husband in 116 cases, husband to wife in 37 cases (30 with previous pregnancies) and in 2 cases from members of clergy-,29 pairs were related in law. 75 patients received non-specific blood transfusion; 175 (95%) pairs showed 3-6 MM. We considered the effects of previous pregnancies and pre-transplant blood transfusions on a possible sensitisation of the recipients. The monitoring group was represented by 313 LRD pairs sharing 1 haplotype. Furthermore we compared a selected group of 46 LURD grafts with 2-4 HLA MM with a 262 cadaver donor transplants group with the same degree of compatibility. The statistical analysis were performed with the χ2 method. Results: the actuarial graft survival of each group is reported in the following table:TableConclusions: In our experience immunological factors, such as HLA compatibility, pre-transplant blood transfusions and previous pregnancies do not influence graft survival; furthermore LURD kidney transplantation is a safe procedure and can be considered an effective and alternative source to compensate cadaver organ shortage.
