Abstract

The November 2022 JADA article titled “Deep Caries Removal Strategies: Findings From The National Dental Practice-Based Research Network” (Jurasic MM, Gillespie S, Sorbara P, et al; for the National Dental PBRN Collaborative Group. JADA. 2022;153[11]:1078-1088) advocates that for selective caries removal “...in deep cavitated lesions, selected removal to soft dentin is preferred, with more carious tissue retained selectively over the pulp.” The concept of incomplete caries removal is in direct conflict with the American Association of Endodontists' position statement on vital pulp therapy1 that advocates complete caries removal and states that “residual caries compromises necessary observations of pulpal inflammation levels and areas of potential necrosis.” With the advent of calcium silicate cements (mineral trioxide aggregate and others), dentists have an option that provides predictable success rates for vital pulp therapy procedures in permanent teeth with symptomatic or asymptomatic irreversible pulpitis.
