A positive chloride: phosphate ratio (Cl/PO4) of more than 33, as indicated in the brief communication by Palmer, Nelson, and Bacchus in the February issue (Ann Intern Med 80:200-204, 1974) was found in 7 of our 11 proved cases (Table 1) of primary hyperparathyroidism without renaldisease. Six of these patients had one or more parathyroid adenomas; the seventh had parathyroid hyperplasia.
